It was created in 1944 after Colonel Russell "Red" Reeder argued that ground troops needed a medal of their own — airmen had the Air Medal, and he thought the infantry deserved the same recognition for the work they were doing. Roosevelt established it by executive order on 4 February 1944 and made it retroactive to 7 December 1941. Awarded for heroism or meritorious service in a combat zone; when it's for valor specifically, it carries a bronze "V" device.
