Created by the Army in 1965, the EFMB tests field medical skills to a brutal standard — pass rates have run as low as 6-7% in the toughest years, historically averaging somewhere between 8% and 30% depending on the fiscal year. Any medical MOS can attempt it, but few succeed; it's widely considered one of the hardest special skill badges in the entire Army to earn.
